The Challenger Disaster 27 Years On

Yesterday was the 27th anniversary of the explosion of the Space Shuttle Challenger, caused by the failure of a frozen rubber o-ring in one of its booster rockets. I was not going to posted on it, and I might have taken the easy route and posted Richard Feynman's famous appendix to the official report on the disaster in which he demonstrates the causes, briefly, and discusses the bureaucratic role of management in underestimating the risk and pushing forward the launch.
